What is Direct Primary Care (DPC)?
Direct Primary Care (DPC) is a healthcare model that focuses on a direct relationship between patients and their primary care providers.
With DPC, patients pay a monthly membership fee that covers most primary care services, eliminating the need for copays and insurance billing for routine visits.
This model allows for more personalized care, longer appointments, and better access to your healthcare provider.

Why is there a membership fee?
Your membership fee covers the cost of your appointments as well as a direct line to your provider with appointments often having same day or next day availability. As well as some after hours.

Is there a registration fee?
Yes, each patient is required to complete an application, which will be reviewed by the Revitalize Health & Body staff. Upon acceptance, there will be a one-hour new patient visit to discuss your medical history, needs, and medications.
An initiation fee is $199 for single individuals or $299 per couple or family that also includes lab work.

Which payment types do you accept?

Do I have to pay a copay?
No copays here!
As a member of Revitalize Health & Body Direct Primary Care, all your in-office care is included in your monthly membership fee. You can visit us as often as you need for any health concern without worrying about additional charges.

Do I still have to pay my monthly membership fee even if I don't need medical help?
Yes, the monthly membership fee is required regardless of whether you utilize medical services during that time. This fee ensures that you have continuous access to care when needed.
Do I have to sign a contract or make a long-term commitment?
You will be required to sign a DPC patient agreement annually, however, with a 30-day written notice you may cancel your membership. This allows us to ensure a smooth transition and proper continuity of care.

Is Revitalize Health & Body Direct Primary Care a form of insurance
or an insurance substitute?
No, RH&B Direct Primary Care is not insurance or an insurance substitute.
It’s a membership-based healthcare model where you pay a monthly fee for direct access to your doctor and a range of services without the need for insurance claims or co-pays.

Will you file insurance claims?
No, we do not file insurance claims. However, a superbill can be provided upon request after each visit. It is the patient's responsibility to file any necessary authorizations and/or claims with their insurance provider.

Do you accept Medicaid/Medicare?
No, we do not accept Medicaid or Medicare. Individuals who are currently on Medicare are not eligible to apply for the program.

I have health insurance. Why should I pay an additional monthly fee for your service?
Having health insurance is valuable, but it often comes with limitations like long wait times, rushed appointments, and unexpected copays. Our Direct Primary Care (DPC) service provides you with personalized, accessible healthcare without the usual insurance hassles.
For a predictable monthly fee, you gain unlimited office visits, direct access to your provider, and comprehensive care tailored to your needs. It’s about investing in a higher level of care, where your health is our priority, not the insurance company's bottom line.

Can I use my health insurance at other healthcare providers if I need diagnostic tests, must go to the ER, see a specialist, or require hospitalization?
Yes, you can still use your health insurance at other healthcare providers for diagnostic tests, ER visits, specialists, or hospitalizations. If needed, you will receive a referral to an outside facility.
In case of an emergency, no referral or permission is required to go to a hospital.

I don't have health insurance. Can I still be a member?
Yes, you can still be a member even without health insurance. However, having health insurance is highly recommended for emergencies, continuity of care, lab work, imaging, and other medical needs outside of what DPC covers.

Are you accepting new patients?
Yes, we are accepting new patients. All prospective patients are required to complete an application, which will be reviewed by our team. Upon acceptance, you'll schedule a one-hour new patient visit to discuss your health needs, history, and medications.

Do you see children?
Yes, the practice sees patients ages 10 and up currently.

Can I walk-in or do I need an appointment?
Appointments are required, but we do offer same-day or next -day appointments to accommodate your needs.

Can a schedule an appointment online, via email or text?
Yes, you can conveniently schedule an appointment online, via email, or through text.

How long does a typical visit last?
A typical visit lasts between 20 to 60 minutes, depending on the patient's needs.

How early should I arrive for my first appointment?
Please arrive 15 minutes early for your first appointment in case there is any additional information we need from you.

What should I bring to my first PT appointment?
For your first PT appointment, please bring the following:

What are virtual visits?
Virtual visits are appointments with a healthcare provider that take place via phone or video instead of in person.
They can be conducted from a smartphone, tablet, or computer, and patients can access care from home, work, or anywhere else they have an internet connection.

Can I speak to a healthcare provider even when I'm at work, out of town, or on vacation?
Yes, you can speak to a healthcare provider even when you're at work, out of town, or on vacation.
Do you make house calls?
No, unfortunately, we do not make house calls.

Are procedures included in my membership?
Yes! All many in-office procedures such as skin tag removal, suture removal, and more, are included in your monthly service fee with no additional copays.
IV Drips and injections are subject to additional costs but DPC members do receive 10% of these services.

What if I need lab work? Is that covered?
Lab work is conveniently drawn in-house. Your membership includes an annual panel at no extra cost. If you need additional labs, they can be purchased separately at your request.

Will you have access to my old records from my previous provider?
Yes. You can complete a medical records request form in our office, and we'll take care of faxing it to your previous provider's office to obtain your records.

Will you be able to manage my care if I'm admitted to the hospital?
While we won’t be able to directly manage your hospital care, we can communicate with the hospitalist, specialists, and nursing staff involved in your treatment.

Will you share my medical records with my insurance company, the government, or other healthcare providers?
Your medical records will only be shared with other healthcare providers when necessary for continuity of care. Additionally, records may be released if required by law, such as in cases of communicable diseases or subpoenas.
Will you have access to my hospital records if I'm admitted?
Absolutely. Once you complete a medical records request form in our office, we can have your records sent over electronically from the hospital or medical facility. This allows us to review and store them securely in your chart.

I'm a healthy person, and I rarely need medical care.
How would this plan benefit me?
This plan is a great option for you.

Did you mention discounted medications?
Yes, we’re able to carry a limited supply of in-house.
Imagine needing a common medication, like blood pressure medication, antibiotic, or ear drops and not having the time to visit the pharmacy.
You can pay a discounted price and leave our office with your medication.
Most 30-day supplies are just $15, and 90-day supplies are $40.
