At Revitalize Health & Body we know weight loss can be a struggle at times and so we are here to help you shed some pounds and get healthy! We offer customized weight loss plans that start with in depth dive into blood work, lifestyle, comorbidities and medical history. Plans also include nutrition and exercise programs, twice monthly check-ins, direct access to your provider as well as the use of peptide therapy, phentermine, or the popular compounded medications.

New Patient Consultation $250

Includes:

In depth review intake forms, past medical history, medications and past blood work

Assessment of vital signs, starting weight, measurements and progress pictures.

A custom plan for physical activity & caloric intake, based on resting metabolic rate, will be developed as well as weight & health goals.

New blood work will be ordered (costs of labs included in $175) and we will discuss which membership plan best suits your goals.

Appointment will last up to 60 minutes

*If completing a virtual appointment, you will need access to a scale & tape measure. We will discuss how to take progress photos.
